How We Do It

Consultation
We begin by meeting with you and your loved one(s). During this initial meeting we

  • fully discuss our services, our fees, and our professional accountability (ethics)

  • review your accountability to us and to your loved one.

  • Please note:We view the older adult as our primary client–no matter who is paying for our services. The client is central to all we do throughout our management.

    Stage I
    If we decide to proceed, we

  • conduct a comprehensive assessessment, and

  • write a plan of care.

  • Next, we meet with the client and designated others to discuss the plan and the suggested solutions. At this point we are available to continue working with the client. If our continued involvement is needed, we move to Stage II.

    Stage II

    You will immediately notice that life is easier for all involved! Caregivers (formal and informal) are involved as much or as little as they choose. That is, we can manage all the care or we can support you as you mange the care. During this stage we

  • implement the solutions in the management plan of care
  • organize and coordinate care activities
  • monitor the well-being of your loved one (and you), and
  • evaluate and monitor the older adult’s response to the plan, making changes accordingly.
  • Stage III

    At this point the client’s care has likely stablized. We discuss whether our continuing involvement is of benefit to all concerned.
